Hope Care in the European agenda for territorial, integrated care: The recently presented telemonitoring project led by CIM Viseu Dão Lafões, in articulation with ULS Viseu Dão-Lafões, Social Security and local social sector partners, is a strong example of this agenda becoming real at territorial level.
